The Post-diagnostic Support (PDS) and Care Co-ordination Improvement Programme 2025-2026 is now open. Expressions of interest are encouraged from teams seeking improvement and implementation support.

We welcome expressions of interest to join our improvement programme by Friday 28 March 2025 at 5pm.
You can submit your expression of interest by using this MS Form.

Previous learning
The programme will build upon the success and learning from our previous programmes on dementia.
It is aligned with:
- Scottish Government’s Dementia Strategy,
- Dementia SIGN Guideline 168,
- Getting it Right for Everyone programme
It will also support the implementation of the Ageing and Frailty Standards (2024).
