The programme will launch in April 2025 and run for 12 months.
Programme delivery
Our multidisciplinary team will support participating teams to identify, test and spread evidence based change ideas.
Support includes:
- One-to-one quality improvement coaching.
- Places at two in person learning sessions.
- Site visits from the Healthcare Improvement Scotland team.
- Access to clinical and improvement expertise on frailty.
- Advice on data and measurement, strategic planning and engaging people and families in improvement.
Teams will be expected to:
- Present their learning to other teams who are part of the programme.
- Complete an operational agreement between themselves and Healthcare Improvement Scotland.
- Complete a project charter.
- Test change ideas at pace.
- Submit monthly data and progress reports.
- Attend two in-person learning sessions and a series of online events.
Joining criteria
The programme is open for expressions of interest from NHS boards and/or health and social care partnership teams. The teams should be seeking improvement and implementation support focused on:
- setting up hospital front door frailty pathways
- improving integrated care coordination across primary, community and acute care
GP practices and partner organisations, for example third sector bodies, may also join the programme. You should have support from your local health and social care partnership or NHS board.
Teams must fulfil the following criteria:
- Be able to identify a project team. This should include a project coordinator and clinical/professional lead.
- Have organisational support to participate in the programme. This includes support from the relevant executive sponsor and alignment to strategic organisational priorities.
- Key members of the team must be able to dedicate sufficient time to deliver a quality improvement project over the 12-month duration of the programme. This will include attending a mixture of sessions with our coaching team and time to work on the project within your team.
Selection process
A panel from Healthcare Improvement Scotland will assess all expressions of interest. Teams will be notified of the outcome by 10 April 2025.
Successful teams will be expected to attend an online welcome session at 1pm on 14 April 2025. There is also an in person learning session on 21 May 2025.
